Case Study: Working with Families

We support Nathan, who is a young adult with severe learning disabilities and associated complex needs, within our specialist day services. We worked with his family, school, and children’s support provider to ensure that his transition to adult services went as smoothly as possible.

His mum Glenys tells their story:

“I started looking for adult support placements for Nathan while he was in his final year at school. A member of staff at his school recommended the Brothers of Charity Services in Lancashire.

“During the transition stage, the team at the Brothers of Charity Services worked closely with Nathan, myself, his social work team, children’s support provider and his school to find out as much as possible about him, right down to the finer details of his routine. They made regular visits to Nathan’s school and carried out shadowing sessions during this time.

“The Brothers of Charity Services were a breath of fresh air throughout Nathan’s transition stage. They made sure we had all of the information that we needed, and that we were fully involved and informed along the way.

“Nathan has been supported by the Brothers of Charity Services for 12 months now and he is a bright and happy young man who always has a smile on his face.
